A complete application must be received in the Dental Hygiene office by 5:00pm, February 1, 2009.  Failure to submit all required materials will negate the application process.  It is the applicant’s responsibility to verify that all materials have been received by the Dental Hygiene Program. The above criterion is subject to change without notice.

A student that has been dismissed from the dental hygiene program for academic reasons may reapply to the program only after meeting with the Program Director to develop and successfully complete an Independent Education Plan (IEP) that adequately addresses the issues related to the primary cause of the dismissal. No additional admission points will be awarded for completing the IEP.  However, successful completion of the IEP will enable the student to submit a completed application.

The Dental Hygiene Department Chair makes all final determinations concerning an applicant’s admission file.  Specific questions should be directed to Mark Patterson, Program Director, 1-800-562-1105 ext 247.  General questions concerning the application process can be answered by Kathy LaNoue at 1-800-562-1105-ext 245.
